Schlussfolgerung

Spontane Nierenrupturen sind selten. Beweisend ist nur die Computertomografie, die Sonografie liefert nur im Einzelfall zusätzliche Aspekte.

Wenn kein Trauma eruierbar ist, sollte eine gezielte Anamnese nach einer Antikoagulanzientherapie erfolgen. Daneben gibt es immer eine Pathologie der Niere in Form eines Tumors oder einer PAN.

Deshalb sind spontane Rupturen nicht wie Nierentraumata zu behandeln, welche in den geschilderten Fällen u.U. auch konservativ zu behandeln gewesen wären. Nach unserer Auffassung muss daher in diesen Fällen immer die umgehende operative Freilegung der Niere erfolgen.
