Zusammenfassung

Mit annähernd 500.000 Sterbefällen pro Jahr in den Industrie-nationen der Nordhalbkugel ist und bleibt der Lungenkrebs die häufigste Krebstodesursache — zu 90% durch Zigarettenrauchen bedingt. Die Biologie der Tumoren ist äußerst variabel, histologisch sind Mischtypen mit kleinzelligen Anteilen besonders häufig. Nur 30% aller Erkrankten werden mit kurativer Zielsetzung operiert. Trotz enormer chirurgischer Fortschritte (Letalität der Lobektomie 1–4%) konnte die Langzeitprognose aller Operierten mit ca. 35% 5-Jahres-Überleben nicht relevant verbessert werden. Auch sog. Frühstadien scheinen bereits zellulär zu streuen, sodass die neoad- juvante/adjuvante Chemotherapiestrategie auch in diesen frühen Stadien eine Bedeutung erlangt hat. Un abhängig davon wird auch in Zukunft in weit fortgeschrittenen Stadien die Operation mit palliativer Zielsetzung ihren festen Stellenwert haben.
