Zusammenfassung
Die Nodalstatus ist, nach der R0-Resektion, der wichtigste prognostische Parameter für Patienten mit einer soliden Tumorerkrankung. Obwohl es inzwischen durch tumorbiologische und molekulargenetische Untersuchungen gelungen ist, ein besseres Verständnis der Metastasierung in ihren Einzelschritten zu erhalten, ist die „Pathophysiologie“ der Lymphknotenmetastasierung weiterhin unklar. Bis jetzt gibt es kein zuverlässiges bildgebendes Verfahren, mit dem prätherapeutisch eine valide Aussage zum Lymphknotenstatus eines Patienten getroffen werden könnte (Siewert u. Sendler 2000).
Bei gastrointestinalen Tumoren ist die komplette Resektion des Tumors und seines Lymphabflussgebietes entscheidend für die Prognoseverbesserung. Doch erst die postoperative histopathologische Aufarbeitung des Resektates gibt eine zuverlässige Aussage über den Lymphknotenstatus. Diese Aussage ist abhängig sowohl von der Anzahl der resezierten als auch von der der histopathologisch untersuchten Lymphknoten. In diesem Zusammenhang wurde das Wort „Chirurgen und Pathologen als prognostischer Faktor für den Tumorpatienten“ geprägt (Schlag 2003). Dies bedeutet, dass in jedem Fall - außer bei gesicherten Fernmetastasen - radikal lymphadenektomiert werden muss. Die Indikation zur adjuvanten Therapie ist meistens abhängig vom Nachweis metastatisch befallener Lymphknoten.
Das Konzept des Sentinel-Lymphknotens (engl. sentinel, der Wächter) ist ein diagnostisches Verfahren, mit dessen Hilfe eine recht genaue Aussage über den Nodalstatus zu treffen ist - und dies ohne die Morbidität der radikalen Lymphadenektomie. Es basiert auf der Annahme, dass es für jeden Tumor einen oder mehrere Lymphknoten gibt, die als erste von Metastasen befallen werden. Nach histopathologischer Untersuchung dieser Sentinel-Lymphknoten kann der Lymphknotenstatus des tumorabhängigen Lymphabflusses zuverlässig vorhergesagt werden. Es hat sich in diesem Zusammenhang bei bisher allen untersuchten Entitäten gezeigt, dass auch die Lymphknotenmetastasierung per continuitatem erfolgt, und nicht „de principe“ in allen Kompartimenten besteht. So wurde beim Mamma und beim Magenkarzinom bewiesen, dass es sog. Skip-Metastasen (Überspringen von Kompartimenten) kaum gibt (Veronesi 1997).
Die Hauptziele des Sentinel Lymph Node Mapping sind also die Verbesserung des Stagings durch Detektion der Lymphknoten, in denen sich mit höchster Wahrscheinlichkeit zuerst Metastasen entwickeln, und die Reduktion der Morbidität durch Individualisierung der radikalen Lymphadenektomie.
