Zusammenfassung
Differenzierte Therapieverfahren können das Überleben von Patienten mit malignen Erkrankungen deutlich verbessern, wenn nicht sogar vielfach verlängern. Um eine chirurgische oder minimal-invasive interventionelle Therapie effizient einsetzen zu können, ist eine exakte Diagnostik von Art und Lokalisation des Primärtumors sowie seiner möglichen Tochterabsiedlungen notwendig.
Die bildgebenden Verfahren haben die klassische klinische Untersuchung extrem erweitert. So erlaubt die gerade in den verschiedenen Schnittbildverfahren ständig verbesserte Orts- und Kontrastauflösung einen immer detaillierteren Einblick in Organstrukturen. Moderne CT-Scanner ermöglichen zeitlich hochaufgelöste Aufnahmen des gesamten Körpers einschließlich der Gefäßdarstellung (CT-Angiographie), wobei die örtliche Auflösung bis in den Submillimeterbereich reichen kann. Die aktuellen Hard- und Softwareentwicklungen der Magnetresonanztomographie gestatten zudem, z. B. durch den Einsatz von gewebespezifischen (funktionellen) KM oder durch den Einsatz von Perfusionmessungen, über die morphologische Darstellung hinaus äußerst differenzierte pathologisch-funktionelle Aussagen.
Abhängig von der Fragestellung zeigen die verschiedenen radiologischen Methoden eine sehr unterschiedliche diagnostische Wertigkeit. Um ein optimales Untersuchungsergebnis zu erhalten, muss nicht nur das am besten geeignete Untersuchungsverfahren ausgewählt werden, sondern auch die Untersuchung auf die spezifische Fragestellung abgestimmt werden. Voraussetzung hierfür ist eine enge Kooperation zwischen Diagnostiker und Kliniker, wobei für den Kliniker ein grundsätzliches Verständnis radiologischer Techniken wichtig ist, um eine bildimmanente Information, die über die anatomische Information hinausgeht, zu verstehen. Je spezifischer dann die Fragestellung formuliert wird, um so besser lässt sich das bildgebende Verfahren an das diagnostische Problem anpassen.
