Zusammenfassung
Das Tubenkarzinom macht etwa 0,3% aller gynäkologischen Malignome aus. Im Gegensatz zum Ovarialkarzinom bewirkt es häufig eine frühere klinische Symptomatik, weshalb Tubenkarzinome öfter in früheren Stadien entdeckt werden. Es breitet sich über das Peritoneum und bevorzugt lymphogen aus. Die lymphatische Dränage der Tube erfolgt in erster Linie via Ligg. infundibulopelvica. Bei bis zu 40% der Frauen treten paraaortale Metastasen ohne gleichzeitigen Befall der pelvinen Lymphknoten auf.
Eine maximale zytoreduktive Operation steht, analog wie beim Ovarialkarzinom, im Mittelpunkt des Therapiekonzepts. Die Chemotherapie des Tubenkarzinoms erfolgt wie beim Ovarialkarzinom mit platinhaltigen Kombinationen. Bei der Überwachung des Krankheitsverlaufs ist v. a. die Beachtung klinischer Symptome und des Tumormarkers CA-125 wertvoll.
