Zusammenfassung
Zu kaum einem anderen Zeitpunkt sind Zufuhr und Utilisation der Nahrung von größerer biologischer Bedeutung als während der frühen kindlichen Entwicklung (Koletzko 2004). Während die Nährstoffzufuhr beim Erwachsenen lediglich den Erhaltungsbedarf für die Organfunktionen und den zusätzlichen Bedarf für körperliche Arbeit decken muss, kommt beim Kind der hohe Energie- und Substratbedarf für das Körperwachstum hinzu. Das enorm rasche Wachstum, mit einer Verdopplung des Körpergewichtes reifer Neugeborener in nur 4–5 Monaten nach der Geburt (bei Frühgeborenen sogar in nur ca. 6 Wochen) und auch die rasante Differenzierung der Gewebe und Organe hängen von einer sehr hohen Nährstoffzufuhr pro kg KG ab. Ein junger Säugling benötigt v. a. aufgrund seiner hohen Wachstumsgeschwindigkeit im Vergleich zum Erwachsenen pro kg KG fast die dreifache Energiezufuhr. Eine marginale oder im Verhältnis verschiedener Nährstoffe untereinander unausgewogene Substratzufuhr ist bei einem wachsenden, sich entwickelnden Organismus sehr viel kritischer als bei Erwachsenen in einer Gleichgewichtssituation, zumal Säuglinge nur sehr begrenzte Kompensationsmöglichkeiten haben: Einerseits sind nur geringe körpereigene Nährstoffreserven vorhanden, andererseits besteht eine Unreife verschiedener Stoffwechselwege (z. B. Aminosäurestoffwechsel: Cystein kann im frühen Lebensalter nicht endogen synthetisiert werden und wird zur essenziellen Aminosäure) und physiologischer Homöostasemechanismen (z. B. Nierenfunktion: Die geringe Fähigkeit zur Urinkonzentrierung bei jungen Säuglingen führt zu hoher Anfälligkeit gegen niedrige Wasser- bzw. hohe Salzzufuhr). Entsprechend treten bei gesunden, aber auch bei chronisch kranken Kindern sehr rasch klinisch offensichtliche Auswirkungen einer unausgewogenen Ernährung auf, deutlich erkennbar z. B. durch eine gestörte Gewichts- und Längenzunahme. Die Qualität der Ernährung beeinflusst zudem Wachstum, Differenzierung und Funktionen einzelner Organsysteme, wie z. B. des Nervensystems.
