Summary
The role of patellar replacement in total knee arthroplasty is controversial. With a modern patella-friendly femoral component design, patellar resurfacing is frequently not necessary. In the revision situation, the most difficult and unresolved question is what to do with an eroded avascular patella following patellar component loosening. This is never an issue if the patella was not resurfaced at the index operation.
