Zusammenfassung
Dauert ein akuter Schmerz trotz jeweils indizierter medizinischer Behandlungsmaßnahmen länger als 6 Monate an, sprechen wir gemäß einer internationalen Konvention von einem chronischen Schmerz. Zu den häufigsten chronischen Schmerzsyndromen zählen verschiedene Kopfschmerzformen, einschließlich Migräne, sowie chronische Rückenschmerzen bei rheumatischen und degenerativen Erkrankungen. Chronische wirbelsäulenbedingte Rückenschmerzen stellen nach einem meist langjährigen Krankheitsverlauf mit 25% den häufigsten Grund für Frühberentungsmaßnahmen bei Frauen, mit 32% den zweithäufigsten bei männlichen Arbeitnehmern (Hagen et al. 1997). Die Entwicklung effektiver Behandlungsmaßnahmen setzt Kenntnisse darüber voraus, bei wie vielen Menschen ein einmalig auftretender, akuter Schmerz in ein chronisches Schmerzleiden übergeht und welche Faktoren zu einer lang dauernden Chronifizierung beitragen. Ein empirisch gesichertes Wissen zu diesen Fragen existiert gegenwärtig vor allem im Bereich chronischer, wirbelsäulenbedingter Rückenschmerzen.
Epidemiologischen Studien zufolge leiden 80% der Bevölkerung einmal in ihrem Leben unter akuten Rückenschmerzen (Berger-Schmitt et al. 1996; Brown et al. 1998). In etwa 10% der Fälle zeigt sich bereits bei ersten Behandlungsversuchen eine anhaltende Therapieresistenz. In den übrigen 90% tritt bei ersten Behandlungsversuchen (z. B. Entlastung, Physiotherapie, Analgetika) kurzfristig eine Besserung ein. Langfristig jedoch weisen etwa 35% chronisch anhaltende oder rezidivierende Beschwerden auf (Biering-Sorensen 1983; Croft et al. 1999; Waddell 1998). So wiesen beispielsweise in einer Stichprobe von 494 Patienten mit chronischen Schmerzen am Johns Hopkins Hospital in Baltimore 85% Zeichen von Depressivität auf (»major de pression« nach DSM III - R, Long 1988).
