Zusammenfassung
Die chronische Pankreatitis ist durch die schubweise oder kontinuierliche Zerstörung des Pankreasgewebes im Rahmen der chronischen Entzündung definiert. Nach einer subklinischen Phase unterschiedlicher Dauer kommt es meist zu rezidivierenden akuten Schüben und zur exokrinen und endokrinen Pankreasinsuffizienz. Die Ätiologie ist in den Industrienationen in 80% der Fälle langjähriger Alkoholmissbrauch.
Die Therapie ist rein symptomatisch: Alkohol- und Nikotinkarenz sowie eine suffiziente Schmerztherapie z. T. unter Einsatz invasiver Verfahren. Mittels interventioneller Endoskopie sind Gallengang-, Pankreasgang- und Pseudozystendrainagen sowie in der Kombination mit extrakorporaler Stoßwellenlithotripsie die Entfernung präpapillärer Pankreasgangkonkremente möglich. Die interventionelle Endoskopie hat sehr wahrscheinlich keinen Einfluss auf den Langzeitverlauf der Erkrankung. Die Therapie der exokrinen Insuffizienz erfolgt mit Schweinepankreatin mit dem Ziel der Beseitigung/Besserung der Maldigestion. Die Applikation fettlöslicher Vitamine, Einhaltung einer fettrestringierten Diät oder Gabe von mittelkettigen Triglyzeriden ist nur bei therapierefraktärer schwerer Maldigestion indiziert. Die Therapie der endokrinen Insuffizienz kann vorübergehend mit oralen Antidiabetika erfolgen, in der Regel mit Insulin. Eine Indikation zur Operation kann zur Schmerztherapie, Therapie von Komplikationen sowie bei Karzinomverdacht bestehen.
