Zusammenfassung
Wenn sich bei Patienten mit rezidivierenden Symptomen eines mechanischen Dünn- bzw. Dickdarmileus durch Ausschöpfen umfangreicher diagnostischer Maßnahmen (bis hin zur Probelaparotomie) das Vorliegen einer mechanischen Obstruktion ausschließen lässt, wird von einer chronisch-intestinalen Pseudoobstruktion (CIPO) gesprochen. Man unterscheidet eine sekundäre CIPO, die auf dem Boden einer definierten Grunderkrankung auftritt (▶ s. unten), von einer primären Form, wo die Genese der CIPO unklar bleibt (»hronischidiopathische intestinale Pseudoobstruktion«). Die Erkrankung betrifft meist den gesamten Gastrointestinaltrakt (einschließlich Ösophagus, Oddi-Sphinkter und Anorektum), in einigen Fällen kann jedoch der Befall eines isolierten Darmabschnitts klinisch ganz im Vordergrund stehen (z. B. »egaduodenum« oder »Megakolon«).
