Zusammenfassung
Die mikroskopische Kolitis ist eine Kolitisform, die endoskopisch nicht zu identifizieren ist und nur histologische Zeichen einer Kolitis aufweist. Es werden 3 verschiedene Hauptformen beschrieben: die lymphozytäre Kolitis, die kollagene Kolitis ohne lymphozytäre Infiltration und eine gemischte Form mit verdicktem Kollagenband und einer Vermehrung intraepithelialer Lymphozyten. Die kollagene Kolitis wurde erstmalig 1976 erwähnt; seither bei Hunderten von Patienten diagnostiziert.
Die neutropene Kolitis wurde 1930 erstmalig in der Literatur erwähnt und ist eine lebensbedrohliche, entzündliche, nekrotisierende Erkrankung des Ileozökalpols, die hauptsächlich beim immunsupprimierten Patienten auftritt. Häufig liegt eine Sekundärinfektion vor (z. B. mit Clostridien, v. a. Clostridium septicum).
Vor 2 Jahrzehnten wurde von Glotzer et al. beobachtet, dass nach Ausschalten eines Dickdarmsegmentes eine Kolitis auftreten kann, die den chronisch entzündlichen Darmerkrankungen und hier insbesondere der Colitis ulcerosa ähnelt. Pathophysiologisch spielt die Ableitung des Darminhaltes (englische Bezeichnung »diversion«) eine entscheidende Rolle, sodass der Begriff Diversionskolitis geprägt wurde.
