Résumé
Le tamoxifène a longtemps été la molécule de référence dans le traitement hormonal du cancer du sein. Il est aujourd’hui surpassé par les anti-aromatases chez la femme ménopausée : quatre études démontrent, par rapport à l’utilisation traditionnelle du tamoxifène pendant cinq ans, un bénéfice sur le risque de récidive et suggèrent une tolérance immédiate plus favorable pour les anti-aromatases. La survie sans récidive est améliorée significativement par l’anastrozole comparé directement au tamoxifène, par l’exemestane et l’anastrozole substitués au tamoxifène deux à trois ans après le début du traitement et par le létrozole comparé au placebo après cinq ans de tamoxifène. Ce dernier, de plus, améliore la survie pour les tumeurs N+. La tolérance immédiate des anti-aromatases est bonne et même, pour l’anastrozole, globalement meilleure que celle du tamoxifène; cependant, la perte minérale osseuse provoquée par la déplétion estrogénique justifie des études spécifiques de protection contre l’ostéoporose. L’anastrozole, le létrozole et l’exemestane diminuent significativement le risque de cancer du sein controlatéral, ce qui motive des études de prévention avec ces molécules dans les populations ménopausées à risque.
