Résumé
Les conséquences des anomalies de torsion des membres inférieurs sur la marche et sur les articulations ont été peu étudiées sur le plan biomécanique. Quelques travaux rapportent des modélisations expérimentales sur pièces cadavériques et, plus récemment, le développement des techniques utilisées dans les laboratoires d’analyse du mouvement ont permis d’effectuer des mesures directement sur le patient. La mesure des forces de réaction au sol, des pressions plantaires et des mouvements articulaires a révélé des modifications d’événements importants du cycle de marche comme la réception ou la fin de phase d’appui dans des configurations de marche avec les pieds orientés vers l’intérieur ou vers l’extérieur. Les intensités et les modalités d’application des forces et des pressions sont modifiées mais également les points d’application, ceci mettant en danger les petites structures osseuses et articulaires du pied. L’angle de progression du pied est fondamentalement modifié pour la marche en rotation interne alors qu’il reste normal, bien que décalé en amplitude, pour la marche en rotation externe. Ces résultats biomécaniques peuvent expliquer, au moins en partie, l’origine de pathologies articulaires relatives à ces anomalies décrites dans la bibliographie médicale.
