Abstract
Epileptic seizures are not an uncommon complication of neuroacanthocytosis. Their pathophysiology is not known, yet possible mechanisms are reviewed here. Antiepileptic drug treatment should be started already after the occurrence of the first seizure of a patient with neuroacanthocytosis. Not only general side-effects must be considered when deciding about a specific drug but also effects upon the movement disorder should be taken into account. Due to the rarity of the condition therapeutic recommendations are necessarily empirical yet carbamazepine and oxcarbazepine appear particularly well-suited.
