Conclusions
Periampullary malignancies are mostly of pancreatic origin. Tumors that obstruct the bile duct early, leading to jaundice, tend to have a more favorable prognosis because they are more likely to be diagnosed at an early stage and therefore are resected. Resection offers the best chance for cure. Adjuvant therapy is not yet optimal. Even with resection, the prognosis with most of these cancers is extremely poor.
