Enucleation and Transduodenal Surgical Ampullectomy for Pancreatic and Periampullary Neoplasms: How I Do It

Enucleation and transduodenal surgical ampullectomy are parenchyma-sparing surgical procedures for the management of benign and premalignant tumors of the pancreas and benign pathologies of the ampulla of Vater including adenomas and stenosis. These procedures offer significant advantages with respect to postoperative pancreatic function when compared to formal pancreatic resections, such as pancreaticoduodenectomy, distal pancreatectomy, or total pancreatectomy. Both enucleation and surgical ampullectomy are technically challenging but can be performed safely by the experienced pancreatic surgeon. In this chapter the indications, perioperative management, and outcomes of enucleation and surgical ampullectomy are discussed, and the techniques for both procedures are described in a detailed stepwise manner.
