Zusammenfassung

Multimodale Therapiekonzepte mit Radio-Chemotherapie bieten: — Hohen Therapienutzen für die (10–40%) Responder. — Kein adäquates Ansprechen lokal/systemisch bei 35–80% der Patienten. — Relativ hohes Therapierisiko (20–70% Komplikationen). — Erheblich eingeschränkte Lebensqualität während der Therapie. Downstaging sollte also nur innerhalb von kontrollierten Studien mit differenzierter Erfassung des lokalen/systemischen Befundes erfolgen. Derzeit ist die Rolle chirurgischer Maßnahmen unklar. Für die Palliation bietet die Endoskopie mit neuen Verfahren (Ar-gonplasmacoagulation, Metallstents) hocheffektive, komplikationsarme u. patientenfreundliche Möglichkeiten.

Summary

Multimodal therapeutic concepts including radio-chemotherapy show: — High therapeutic benefit for the (10–40%) responsers. — No adequate local and/or systemic response in 35–80% of patients. — Relatively high therapeutic risk (20–70% complication rate). — Hardly acceptable quality of life during downstaging. It is concluded that down-staging should be performed only in controlled trials with differentiated staging and control of the local and distant tumor manifestations. Unclear remains the role of surgery. For palliative intentions endoscopy offers new methods (argon plasma coagulation, metal stents) with high effectivity, low complication rate and high comfort for the patient.
