Chirurgische Therapie cystischer Tumore des Pankreas

Zusammenfassung

Schlüsselwörter: Von 1986 bis 2001 behandelten wir 31 Patienten mit zystischen Pankreasneoplasien. Bei 13 Patienten fand sich ein Cystadenokarzinom, hierunter ein Fall eines seltenen serösen Cystadenocarcioms. Bei 10 Patienten war eine kurative Resektion (R0) möglich (Resektionsrate: 76,9%). Nach median 61 Monaten (29 – 144 Monaten) lebten 8 Patienten tumorfrei. 2 Patienten verstarben 2 (Pneumonie) bzw. 36 Monate (Lokalrezidiv) postoperativ. Im letzten Fall hatte das Operationspräparat eine nodal-positive Situation gezeigt. Ein Patient nach Rl-Resektion verstarb 44 Monate postoperativ am Tumorrezidiv und hepatischer Metastasierung. Unsere Daten belegen die günstige Prognose zystischer Pankreasneoplasien nach kurativer Resektion — ein Ergebnis der unterschiedlichen Tumorbiologie verglichen mit den duktalen Pankreaskarzinomen.

Cystic neoplasms of the pancreas: surgical therapy and outcome

Summary

From 1986–2001 we treated 31 patients with cystic neoplasms of the pancreas; 13 patients showed a cystadenocarcinoma including one rare case of a serious cystadenocarcinoma. A curative resection (R0) was possible in 10 patients (resection-rate: 76.9%). After median 61 months (range 29–144 months) 8 patients are alive without evidence of tumour; 2 patients died 2 months (pneumonia) and 36 months (local recurrence), respectively, after operation. The latter case of tumour recurrence showed nodal involvement (pNl) in the Whipple specimen. One patient died 44 months after Whipple procedure with tumour-positive resection margin (Rl situation) from local recurrence and hepatic spread. Our data reflect the good prognosis of cystic neoplasms of the pancreas reported in literature after curative resection — a result of the different biological behaviour compared with ductal adenocarcinome.
