Abstract
Adenocarcinoma of the rectum is a common diagnosis that will be encountered by the majority of practicing general surgeons. It is associated with a complex treatment algorithm that is essential to understand such that we can ensure optimal patient outcome and acceptable locoregional recurrence rates. The treatment paradigm continues to change through active research. It is the purpose of this chapter to provide a basic algorithm for practicing surgeons that will address the majority of patient scenarios. While we do not intend for these recommendations to be prescriptive, they should provide a solid working basis. Of course, there will be scenarios outside the scope of this particular algorithm that could be encountered.
