Consultant Corner: Acute Mesenteric Ischemia

Early recognition and consultation with a surgeon is imperative in preserving and restoring tissue perfusion and decreasing mortality in acute mesenteric ischemia. Multidetector CT has become the gold standard imaging study for diagnosis in the acute care setting. Resuscitation with balanced crystalloid intravenous fluids aids in prevention/correction of acidemia and swift transition to vasopressor therapy when needed for resultant sepsis. A hybrid surgical approach, utilizing endovascular interventions for the vascular lesion and open or laparoscopic bowel evaluation, should be the surgical standard of care.
