External Aortic Ring Annuloplasty to Standardize Aortic Valve Repair

Abstract

Degenerative tricuspid and bicuspid aortic insufficiency are the most common etiologies in Western countries, accounting for approximately 2/3 of the cases of aortic insufficiency in the Euro Heart Survey on valvular heart disease [1–3]. It is characterized by dilation the aortic annulus, sinuses and/or sinotubular junction (STJ) diameters preventing coaptation of pliable leaflets which may also be subjected to prolapse [3–6]. Dilation of the annulus is almost constantly present in annulo-aortic ectasia and dystrophic aortic insufficiency, especially in the setting of a bicuspid valve, where an untreated diameter greater than 25–28 mm has been identified as a risk factor for immediate and long term failure of valve repair, but also in isolated aortic insufficiency (AI) [4–13].
