Mitral Valve Disease and Atrial Fibrillation

Abstract

The case report described by the following work is about a male young patient who suffered symptoms of pain of the epigastric region, dizziness, vomiting, and pain in the back, being analyzed by clinicians who evidenced arterial hypertension and a systolic mitral murmur and being prescribed Atenolol and Furosemide. After that, he was referred to a specialist who diagnosed cardiomegaly, according to a chest X-ray. An echocardiographic study was also performed showing mitral valve insufficiency with signs of fibrosis and fusion of the commissures, with dilation of the left chambers, without any other abnormalities. The conclusion of the study evidenced a double mitral lesion, moderate pulmonary hypertension, moderate systolic dysfunction of the left ventricle, and mild tricuspid insufficiency. Also an electrocardiogram study was performed showing an atrial fibrillation rhythm.

After the conclusion of the diagnosis, the patient was referred to a cardiac surgeon, who indicated surgical treatment. After that the patient was admitted in the hospital to perform the surgical procedure, and then the replacement of the mitral valve was successfully performed. Postoperatively a good evolution of the heart rhythm occurred, becoming a sinus rhythm. It was carried out a new echocardiographic study, which evidenced the mechanical prosthesis normofunctioning and mild pulmonary hypertension and increased left chambers.
