Advanced Imaging Modalities

This chapter describes the advanced imaging modalities used in musculoskeletal radiology, including MRI, ultrasound, and CT. The chapter focuses on MRI, discussing the fundamentals of image acquisition and the differences between T1- and T2-weighted imaging. It also discusses the value of MRI for early detection of findings associated with rheumatic diseases, describing hallmark features of rheumatoid arthritis, seronegative spondyloarthropathies, avascular necrosis, and acute osteomyelitis. The basics of ultrasound technology are explained and examples of common findings are provided, showing the role of ultrasound to aid in both diagnosis of rheumatic disease and guidance of joint injections and aspirations. CT is briefly discussed including its many advantages such as speed, patient comfort, and ability to evaluate soft tissue, joint, or osseous infection. Overall, this chapter summarizes the fundamentals and important clinical uses of the advanced musculoskeletal imaging modalities commonly used in rheumatology.
