A Young Man with Mottled Pigmentation on Trunk

Abstract

A 27-year-old male, from rural area of West Bengal, presented with progressive, asymptomatic, pigmentation of the trunk of 5 years duration, and rough palm and soles of 2 year duration. Arsenic level in urine and in drinking water was found to be high and a diagnosis of chronic arsenicosis was made. Arsenicosis is a chronic multisystem disorder resulting from high level of arsenic in the body. Skin is the predominantly affected organ, but no organ system is spared. Also, there is high risk of developing both cutaneous and systemic malignancies. There is no effective treatment; education and prevention are the main treatment strategies. Periodic life-long surveillance for detection of cancer in chronic arsenicosis patients is mandatory.
