Heart failure (HF) is a complex clinical syndrome in which there is a decrease in cardiac output that results in a decrease in its ability to meet the metabolic demands of the body. It is common and often complicated by multiple co-morbidities and polypharmacy. Management is largely driven by multiple medications and hence pharmacists play an important role in caring for these patients. Initiation and titration of medication are key activities when treating patients with HF, as is follow-up and monitoring.
