Restoration of pre-injury form and function after a significant delay can be very challenging. This can arise as a result of nontreatment, delayed treatment or inadequate surgical repair. It is also more likely to occur in high-energy injuries and those injuries resulting in severely comminuted fractures that require bone grafts. A methodical approach following careful analysis of all components of the defect is essential. This chapter gives an overview of the problem and some of the possible solutions.
