In dit hoofdstuk staat de fase van doelbepaling en plan van aanpak centraal. Doelbepaling is een belangrijke stap, die nadrukkelijk samen met de patiënt en mantelzorger wordt uitgevoerd. Dit hoofdstuk beschrijft de achtergronden en visie op het belang van de doelbepaling en het plan van aanpak. Het hoofdstuk sluit af met een conclusie en verwijzing naar de fase van doelbepaling in deel 2 ‘Praktische toepassing van het EDOMAH-programma’.
