Zusammenfassung
In der Klinik der spinalen Gefäßprozesse steht das „Syndrom des Verschlusses der vorderen Spinalarterie“ sehr im Vordergrund. Dem Ausfall eines umschriebenen Gefäßareales entsprechend, kommt es zu einer relativ plötzlich einsetzenden Paraplegie beider Beine, zu einer dissoziierten Sensibilitätsstörung (häufig nur an der oberen Begrenzung des segmentalen Sensibilitätsausfalles) und zu einer Blasen-Mastdarm-Lähmung. Neben der rein klinischen Kasuistik von Preobrashensky (1904), Margaretten (1923), Ornsteen (1932), Schaller, Robert und Stadtherr (1932), Tureen (1937), Beck (1952), Stegmann (1952), Mair und Druckmann (1953), Girard, Garde und Devic (1954), Skinhoj (1956), Barraquer-Bordas und Muntaner-Marqués (1958) und J. Becker (1958) verweisen vor allem die zugleich anatomisch gesicherten Fälle von Bastian (1883), Spiller (1909), Grinker und Guy (1927), Margulis (1929), Zeitlin und Lichtenstein (1936), P. Vogel und H. H. Meyer (1937), Alajouanine und Hornet (1937), Davison (1937), Louis-Bar (1947), Zeitlhofer (1950), Hoffmann (1955), Bahlmann und Hempel (1956) und Henneaux (1958) auf die Verschiedenartigkeit der zugrunde liegenden Veränderungen der Arteria spinalis anterior, z. B. auf embolische oder thrombotische Verschlüsse, Arteriosklerose, Lues, P. nodosa, Kompression durch Tumor, Bandscheibe oder Kyphoskoliose. Im Gegensatz dazu sollen Ausfälle der dorsalen Spinalarterien zu ausgedehnteren Zerstörungen mit weniger charakteristischen Symptomen führen. Für die noch selteneren spinalen Venenprozesse (Phlebothrombosen und Phlebitiden) galt bislang das gleiche. Der folgende Fall, bei dem klinisch an einen Verschluß der vorderen Spinalarterie gedacht worden war, bei dem aber die Sektion eine spinale Thrombophlebitis ergab, mag zeigen, daß das klinische Bild eines spinalen Gefäßverschlusses nichts darüber auszusagen braucht, ob der arterielle oder venöse Teil der Strombahn betroffen ist.
