Abstract
In general, application of antacids does not lead to notable side-effects since the site of action is not systemic. However, the ability of most preparations to bind metal ions, to form insoluble salts with other ionic compounds, or to alter the gastric pH can lead to marked changes in the absorption of other drugs and hence to their systemic bioavailability. For antibiotics, such as tetra-cycline, there is up to a 90% reduction in bioavailability. Some inhibitory effects have also been noted for beta-blockers, captopril, digitalis, H2-receptor antagonists, iron, nonsteroid antirheumatics and theophylline. For many compounds the dosage of the antacid and the timing of application is important. Drugs taken together with antacids on an empty stomach are more likely to be affected than medication which is applied with an interval of one or two hours between the different preparations.
Zusammenfassung
Die Anwendung von Antacida führt im allgemeinen nicht zu erwähnenswerten Nebenwirkungen, da sie nicht systemisch wirken. Die Fähigkeit der meisten Antacidapräparate zur Komplexbildung mit Metall-Ionen, zur Bildung unlöslicher Salzverbindungen mit anderen ionischen Substanzen sowie zur Veränderung des pH-Wertes im Magen hat jedoch erheblichen Einfluß auf die Resorption anderer Medikamente und damit auf ihre Bioverfügbarkeit. Bei Antibiotika, wie z. B. Tetracyclinen, wird sie bis zu 90 % reduziert. Hemmwirkungen sind auch für Beta-Blocker, Captopril, Digitalis, H2-Rezeptorantagonisten, Eisen, nichtsteroidale Antirheumatika und Theophyllin gefunden worden. Für viele Substanzen sind die Dosierung und die Dosierungszeit entscheidend. Wenn Medikamente und Antacida in nüchternem Zustand gemeinsam eingenommen werden, ist ein größeres Interaktionsrisiko zu erwarten, als wenn die einzelnen Präparate zeitlich versetzt angewendet werden.
