Zusammenfassung

Die Implantation eines Extrakorporalen Life-Support-Systems (ECLS) im Rahmen einer kardiopulmonalen Reanimation (CPR) wird definitionsgemäß als extrakorporale CPR (eCPR) bezeichnet und gilt aktuell als Ultima-Ratio-Maßnahme bei Patienten mit einem Herz-Kreislauf-Stillstand, der trotz Durchführung angemessener CPR-Maßnahmen fortbesteht. Wenngleich sich in der aktuellen Literatur Vorteile der eCPR gegenüber der konventionellen CPR abzeichnen, ist das Niveau der wissenschaftlichen Evidenz, die dem Einsatz der eCPR zugrunde liegt, niedrig. Die Empfehlungen in wissenschaftlichen Leitlinien für den Einsatz der eCPR sind daher sehr zurückhaltend und vage. In einem interdisziplinären Konsensuspapier wurden im Jahr 2019 Kriterien, die für oder gegen den Einsatz der eCPR sprechen, sowie infrastrukturelle, prozessuale und personelle Anforderungen zur Anwendung der eCPR formuliert. Dabei sind insbesondere die ethischen Kriterien von besonderer Bedeutung.
