Zusammenfassung
Insgesamt 6,3 % aller Schwangeren in 2016 hatten einen Diabetes mellitus (DM), davon einen Gestationsdiabetes (GDM) in 5,38 %. Jede Schwangere sollte zwischen der 24. und 28. Schwangerschaftswoche mit einem 75 g-Glukosetoleranztest auf einen GDM untersucht werden. Schwangere mit bekanntem DM sollten bereits präkonzeptionell eine normnahe Blutzuckereinstellung anstreben. Humaninsulin ist die Therapie der Wahl. Eine vorherige Therapie mit Insulin aspart oder Insulin lispro kann fortgesetzt werden. Langwirksame Analoginsuline sollten durch NPH-Insulin ersetzt werden. Orale Antidiabetika und GLP-1-Analoga sind kontraindiziert. Bei Schwangeren mit GDM reicht meist eine diabetesgerechte Ernährung aus. Regelmäßige Blutzuckerkontrollen während der Schwangerschaft sind obligat (mittlere Blutglukose 90–110 mg/dl). Die Raten für Frühgeburten, Fehlbildungen und Frühaborte sind erhöht. Risiken für Neugeborene sind: Hypoglykämien, Elektrolytentgleisungen, Organreifungsstörungen. Innerhalb von 10 Jahren entwickeln 35–60 % der Patientinnen mit GDM einen DM.
