Zusammenfassung
Schmerz ist eine der quälendsten Erfahrungen, er wird immer noch als ein Hauptgrund für den Ruf nach aktiver Sterbehilfe angegeben und gilt am Lebensende geradezu als „Normalität“. Das kann heute nicht mehr stimmen, sind doch mehr als 90 % der Schmerzen am Lebensende mit den in diesem Kapitel gezeigten einfachen Strategien behandelbar. Dabei kommt der Einteilung der Schmerzarten – nozizeptiver oder neuropathischer Schmerz insbesondere unter dem Gesichtspunkt des „total pain“ eine Schlüsselrolle zu. Einfache medikamentöse Strategien greifen zu kurz, eine mechanismenorientierte Schmerztherapie im Rahmen eines umfassenden Gesamtkonzeptes ist zielführend. Neben einer guten Grundlagentherapie mit besonderem Fokus auf psychologischen Verfahren werden auch Spezialkenntnisse vermittelt, wie die Schmerztherapie in der Finalphase, bei Suchtproblemen, der Opioidwechsel, die opioidbedingte Hyperalgesie, invasive Verfahren wie die Periduralanästhesie und die Behandlung des Schmerznotfalls.
