Abstract
Osteochondritis dissecans can be a disabling condition with long-term sequelae. Available data suggests that preservation of the normal anatomy is associated with improved outcomes, provided that the fragments heal. While screws or pegs may be usable with large fragments, most lesions include little subchondral bone and are difficult to fix with such devices. Use of an arthroscopically guided fixation technique with mattress sutures and autogenous iliac crest bone graft may extend the capacity to fix and obtain healing of thinner and smaller OCD lesions.
