Rotator Cuff Pathology in the Older Active Patient

Abstract

Our patient population is aging and continuing activity further into life. “Elderly” patients have greater demands on treatment outcomes and thus our approach to this patient demographic must also adapt. Prior beliefs that chronologic age alone disqualifies patients from surgical intervention is not supported by modern literature. A physiologically anchored approach to patient “age” is becoming more accepted and can guide appropriate management. Rotator cuff pathology management should further be guided by the biology of the tissue and its “healabilty” rather than “reparability.” While conservative management is appropriate in some, surgical interventions can provide distinct advantages and has proven to be successful in patients of advancing age. In this chapter we discuss the natural history, pathophysiology, treatment options and outcomes in the management of rotator cuff disease in patients older than 65 years of age.
