Zusammenfassung
Die herkömmlichen klinischen Verfahren, nach T- und N-Kategorie ein Staging präoperativ vorzunehmen und darauf die Therapieentscheidung zur neoadjuvanten Therapie fußen zu lassen ist problematisch, da die T- und N-Kategorisierung häufig ungenau ist und damit wahrscheinlich ein relevanter Anteil an Patienten neoadjuvant radiochemotherapiert wird, ohne dass sich hieraus ein relevanter onkologischer Vorteil ergibt – bei gleichzeitiger Inkaufnahme der Nebenwirkungen der neoadjuvanten Therapie. Die präoperative Indikationsstellung zur primären Operation bzw. neoadjuvanten Chemotherapie, die auf einer präoperativen MRT basiert, stellt deshalb eine attraktive Alternative dar, um unnötige Nebenwirkungen einer neoadjuvanten Therapie zu vermeiden bei gleichzeitig onkologisch vergleichbaren Ergebnissen.
