Zusammenfassung
Operationen in laparoskopischer (LSK) Technik beinhalten spezifische anästhesiologische Risiken. Durch die Anlage des Pneumoperitoneums kommt es zu sich gegenseitig beeinflussenden Veränderungen von Hämodynamik und Atemmechanik. Beatmungsspitzen- und -plateaudrücke nehmen bis zu 40 % zu, in gleichem Maß nimmt die Compliance ab. Hämodynamische Auswirkungen des Pneumoperitoneums sind charakterisiert durch Abfall des Herzzeitvolumens, Anstieg des peripheren und pulmonalen Gefäßwiderstands sowie Erhöhung des arteriellen Blutdrucks. Verstärkt werden diese Effekte durch operationsbedingte Lagerungstechniken wie die Kopftieflage bei der LIHP oder die Anti-Trendelenburg-Lagerung bei der LSK Hiatushernienplastik.
