Zusammenfassung

Postoperative Briden und Adhäsionen sind die häufigsten Ursachen für einen Dünndarmileus. Es finden sich kolikartige Bauchschmerzen, rezidivierendes galliges Erbrechen, eine Zunahme des Bauchumfangs und eine Schocksymptomatik. Die Diagnose wird gestellt durch ein Abdomenleerröntgenbild in Rückenlage und ggf. MRT oder Kontrastmittel-CT oder diagnostische Laparoskopie. Die konservative Behandlung besteht aus Flüssigkeitssubstitution und ggf. Kontrastmittelgabe über eine Magensonde. Die chirurgischen Therapiemöglichkeiten sind die laparoskopische Bridendurchtrennung und Adhäsiolyse, die Laparotomie mit Adhäsiolyse oder eine Darmresektion mit oder ohne Stomaanlage.
