Zusammenfassung
Patienten nach gefäßchirurgischen Operationen/ Interventionen sind nicht nur dem Risiko eines Verschlusses der Rekonstruktion ausgesetzt, sondern auch einem erhöhten Risiko für Schlaganfall, Herzinfarkt oder vaskulären Tod.
Die gerinnungsaktiven Substanzen müssen deshalb angepasst an das individuelle Blutungsrisiko und das Risiko des Verschlusses der Rekonstruktion eingesetzt werden.
Die Absicherung durch Evidenz Studien der Klasse I ist in der Regel nicht gegeben, so dass ein individuelles Konzept mit den Patienten besprochen werden muss, der insbesondere auch über Nebenwirkungen informiert werden muss.
Für das Blutungsrisiko können der HAS Bled Score oder der Atria Score verwendet werden. Die Behandlung mit Vitamin K Antagonisten (therapeutische Antikoagulation) bleibt Sondersituationen vorbehalten (RE-Verschluss, pedale Rekonstruktionen). Auch venöse kniegelenksübergreifende Rekonstruktionen können mit TAH behandelt werden, insbesondere bei Aneurysma Patienten oder Patienten mit guten Abstromgefäßen.
