Zusammenfassung

Akne und Rosazea zählen zu den häufigsten entzündlichen Hauterkrankungen, die eine konsequente Behandlung erfordern. Aktuelle pathogenetische Erkenntnisse erklären neue präventive und innovative Therapienansätze. Bei Acne vulgaris wurden Ernährungsfaktoren als wichtige Auslöser gesichert wie vermehrter Konsum von Milch und Kohlenhydraten, die den mTORC1-Signalweg und damit die Talgproduktion steigern. Endoplasmatischer Retikulum-Stress erklärt die klinischen Triggerfaktoren der Rosazea und erlaubt ein neues Verständnis der Rosazea. Die Acne inversa ist keine Akne im eigentlichen Sinn, sondern ist eine Entzündung terminaler Haarfollikel (dissecting terminal hair folliculitis). Primär besteht eine erhöhte Fragilität des Akroinfundibulums infolge verminderter Notch-Signale sowie eine Verbindung zu autoinflammatorischen Prozessen. Die Kenntnis dieser neuen Zusammenhänge ermöglicht moderne und zielgerichtete Therapien dieser häufigen, psychosozial oft stark belastenden Hautkrankheiten.
