Zusammenfassung
Ösophaguskarzinome sind Erkrankungen des Erwachsenen und zeigen eine zunehmende Prävalenz mit steigendem Lebensalter. Plattenepithelkarzinome entstehen vorwiegend auf der Grundlage jahrelangen Tabak- und Alkoholkonsums; Adenokarzinome finden sich assoziiert mit Adipositas und chronischer gastro-ösophagealer Refluxkrankheit. Die kurative Behandlung des Ösophaguskarzinoms erfolgt multimodal in den meist bei Diagnosestellung anzutreffenden lokal fortgeschrittenen Stadien. Die Therapie-assoziierten Risiken sind dabei besonders zu beachten. Ein exzellentes Risikomanagement, gerade bei Begleiterkrankungen und eingeschränkten Organfunktionen ist von großer Wichtigkeit. Dies setzt gerade bei älteren Patienten eine große Erfahrung und Expertise des Zentrums voraus. Die wissenschaftliche Evidenz für die Behandlung von Ösophaguskarzinomen im höheren Lebensalter ist noch dürftig.
