Pathologie der sekundären Harnabflußstörung im Bereich der Niere und der oberen Harnwege

Summary

The pathology of secondary disturbances in urine flow in the region of the kidneys and the upper urinary tract.

The question of the patho-physiological relationship between distortion and loss of function and of the restoration-capacity of the organ after elimination of the disturbance in flow is very important for the clinical treatment of urine-retention kidney. The slowly-progressing parenchyma damage with dedifferentiation and atrophy of the tubular epithelium is the result of energetic insufficiency.

In this, reduced blood-flow has considerable pathogenetic significance. Isolated nephrons may, however, remain intact even after some weeks. Reduced tubular fluid flow is demonstrable here. The mitotic activity in urine-retention kidney is the same as in compensatory hyperplasia after nephrectomy. Here, we have all the necessary preconditions for functional restoration of the organ (probably only partial) after elimination of the disturbance in flow.

Zusammenfassung

Für die Klinik der Harnstauungsniere ist die Frage nach dein patho-physiologischen Zusammenhang zwischen Form- und Funktionsverlust und nach der Wiedererholungsfähigkeit des Organs nach Beseitigung der Abflußstörung von Bedeutung. Der langsam fortschreitende Parenchymschaden mit Entdifferenzierung und Atrophie des Tubulusepithels ist Folge einer energetischen Insuffizienz. Der reduzierte Blutdurchfluß ist dabei von wesentlicher pathogenetischer Bedeutung. Einzelne Nephrone können jedoch auch noch nach Wochen intakt bleiben. Ein reduzierter tubulärer Flüssigkeitsstrom ist hier nachweisbar. Die mitotische Aktivität in der Harnstauungsniere entspricht der der kompensatorischen Hyperplasie nach Nephrektomie. Damit sind auch die Voraussetzungen für eine — z. T. jedoch nur mehr partielle — Wiedererholung des Organs nach Beseitigung einer Abflnßstörung gegeben.
