Die Strahlenbehandlung des Bronchialcarcinoms

Summary

The conditions for radiotherapy of bronchial carcinomas are so unfavourable that primary radiotherapy should only be used for inoperable cases. Nevertheless, it was possible in 294 patients to achieve a one-year survival rate of 29% with high-voltage therapy. This is more than three times that achieved with conventional deep radiotherapy (200 kV). In patients where it was possible to apply a dose of more than 6000 R to the tumor 54% were alife one year later and 16% were alife 3 years later.

Exploratory thoracotomies considerably decreased the prognosis of subsequent radiotherapy and they should be reduced to an unavoidable minimum.

Postoperative radiotherapy after lobectomies or pneumectomies for bronchial carcinomas is not considered suitable as a routine method and it is only believed to be indicated for special cases.

Zusammenfassung

Voraussetzungen für eine Strahlenbehandlung von Bronchialcarcinomen sind so ungünstig, daß nur bei Inoperabilität primär bestrahlt werden sollte. Trotzdem konnte bei 294 Patienten mit der iochvolttherapie nach 1 Jahr eine Úberlebensrate von 29% erreicht werden. Das ist mehr als das 3fache gegenüber der konventionellen Tiefentherapie (200 kV). Von den Patienten, bei denen auf den Tumor eine Dosis über 6000 R appliziert werden konnte, lebten nach 1 Jahr sogar noch 54% und nach 3 Jahren 16%.

Probethorakotomien verschlechtern die Aussichten der nachfolgenden Bestrahlung erheblich und sollten auf ein — natürlich unvermeidbares — Mindestmaß reduziert werden.

Die Nachbestrahlung nach Lob- oder Pneumonektomien wegen eines Bronchialcarcinoms wird als Routinemaßnahme abgelehnt und nur in besonderen Fällen für zweckmäßig gehalten.
