Summary

During the last few years we have revised our vote for and our knowledge of extended choledochoduodenostomy in the treatment of disordered flow of bile and pancreatic juice, when the disorder lies in the distal ducts and at the papilla. This includes complications of these disorders and the use of the operation as secondary intervention. The disadvantages of an anastotnosis between bile duct and duodenum are great; they are the consequences of the retroduodenal blind ending. Transduodenal measures at Oddi’s sphincter are more suitable for the elimination of the pathogenetic effects of retrograde congestion of bile and pancreatic juices. They also correspond more to our physiological concepts.

Riedel’s proposal, made in 1888, namely, to use choledochoduodenostomy as palliative operation and as secondary measure in inoperable carcinoma of the head of the pancreas. is still used solidly in surgery.

Zusammenfassung

Unser positives Votum und Bekenntnis zur erweiterten Choledochoduodenostomie zur Behandlung von Galle-und Pankreassaftabflußstörungen im distalen Gangapparat und an der Papille sowie ihren Komplikationen auch als Sekundäreingriff haben wir in den letzten Jahren revidiert. Die Nachteile einer Anastomose zwischen Gallengang und Zwölffingerdarm sind groß und ergeben sich aus den Folgen des retroduodenalen Blindsackes. Die transduodenalen Maßnahmen am Sphincter Oddi sind geeigneter, die pathogenetischen Auswirkungen bei Galle-und Pankreassaftrückflußstauungen zu beseitigen, und entsprechen außerdem unseren physiologischen Vorstellungen.

Der Vorschlag Riedels im Jahre 1888, die Choledochoduodenostomie als Palliativeingriff beim inoperablen Pankreaskopf-Carcinom auch als sekundäre Maßnahme zu praktizieren, ist auch heute noch fester Bestandteil unseres operativen Handelns.
