Die Leistungsfähigkeit der percutanen transhepatischen Cholangiographie vor Wiederholungseingriffen an den Gallenwegen

Summary

Intravenous cholecysto-cholangiography intended for the visualization of the bile ducts does not work in jaundiced patients with serum bilirubin values over 2 mg-%. The method often fails in non-jaundiced patients with bili-digestive anastomoses and incomplete outflow of bile from the common bile duct. In these cases exact diagnosis may be achieved by percutaneous transhepatic cholangiography. The surgeon will be able to plan the operation by clarifying before operation the site of the occlusion and the cause in cases of extrahepatic occlusion, both complete and imcomplete. He will be able to forgo intraoperative diagnostic measures. which are difficult and time-consuming, especially in patients that have been operated on before and have scars. If it can be demonstrated that the afferent bile ducts are open, the patient will be spared a laparotomy. Serious complications need not be feared if the ordinary rules applicable to injection and blood coagulation factors are observed.

Zusammenfassung

Die i.v. Cholecysto-Cholangiographie zur Gallenwegsdarstellung versagt bei ikterischen Patienten mit Serumbilirubinwerten über 2 mg-%. Die Methode scheitert auch oft bei anikterischen Patienten mit bili-digestiven Anastomosen und inkompletten AbfiuBstörungen im Bereich des Hepatocholedochus. In diesen Fällen ermöglicht die percutane transhepatische Cholangiographie meist eine genaue Diagnose. Durch die präoperative Klärung der Verschlußlokalisation und der Ursache beim extrahepatischen kompletten und inkompletten Verschluß hat der Chirurg die Möglichkeit, die Operation vorzuplanen und kann auf intra-operative diagnostische Maßnahmen verzichten, die besonders bei voroperierten Patienten im vernarbten Gebiet schwierig und zeitraubend sind. Beim Nachweis von freien ableitenden Gallenwegen bleibt dem Patienten die Laparotomie erspart. Bei Beachtung der Punktionsregeln und der Blutgerimnmgsfaktoren sind schwerwiegende Komplikationen nicht zu befürchten.
