Summary
Of 6 patients who showed a clinically demonstrable hemorrhagic diathesis due to multiple injuries and severe hemorrhagic-traumatic shock, three were treated with antifibrinolytic agents, Cohn-fraction I and Konaktion. The other three patients were treated with Heparin (10 to 20000 LT/25 hours). In addition to the customary laboratory tests the following values were determined every day: Antithrombin, thrombokinase, formation test, factors II, V, VII, VIII and X, thromboelastogram, thrombin time-antithrombin II, platelet count, fibrinogen. In all cases it was possible to demonstrate a defibrinisation syndrome as a typical sign of disseminated intravascular coagulation (DIC). 24 hours after the administration of Heparin the coagulation values already showed a tendency to return to normal. Additional hemorrhages were not observed. The accident victims who were treated with Heparin survived. The other accident victims died on the 3rd day after the accident and/or after the surgical procedure.
Even though, considering the small number of patients, the fact of survival cannot be regarded as a criterion for the favourable action of Heparin, the effect of Heparin on the DIC can nevertheless be determined from the course of the coagulation values. Combined treatment with Heparin and antifibrinolytic agents is only indicated for confirmed secondary fibrinolysis.
Zusammenfassung
Von 6 Patienten, die mit multiplen Verletzungen und einem schweren hämorrhagisch-traumatischen Schock eine klinisch faßbare hämorrhagischc Diathese aufwiesen, wurden 3 mit Antifibrinolytica, Cohn-Fraktion I und Konaktion behandelt, die übrigen 3 mit Heparin (10–20000 E/24 Std). Neben den üblichen Laboratoriumsuntersuchungen wurden folgende Werte täglich bestimmt:
Antithrombin, Thrombokinase-Bildungstest, die Faktoren II, V, VII, VIII und X, Thrombelastogramm, Thrombinzeit-Antithrombin II, Thrombocytenzahl, Fibrinogen. ln allen Fällen konnte ein Defibrinierungssyndrom nachgewiesen werden als typisches Zeichen einer disseminierten intravasalen Coagulation (DIC). Schon 24 Std nach Gabe von Heparin ließen die Gerinnungswerte eine Tendenz zur Normalisierung erkennen. Eine zusätzliche Blutung wurde nicht beobachtet. Die Heparin-behandelten Unfallverletzten überlebten, die übrigen verstarben am 3. Tag nach dem Unfall und/oder nach dem chirurgischen Eingriff.
Wenn auch die Tatsache des Überlebens in Anbetracht der kleinen Zahl nicht als Kriterium für den günstigen Einfluß des Heparins zu werten ist, so läßt sich doch am Verlaufsprofil der Gerinnungswerte die Wirkung von Heparin auf die DMC ablesen. Nur bei nachgewiesener, sekundärer Fibrinolyse ist eine kombinierte Therapie mit Heparin und Antifibrinolytica indiziert.
