Chirurgische Erstversorgung am Unfallort

Summary

Surgical treatment at the very location of an accident presents a small, yet important sector of a modern casualty service. The indications for the employment of a surgeon are life-endangering conditions. The measures used are: 1. respiratory resuscitation, 2. cardiac resuscitation, 3. circulatory resuscitation. The organization should be adapted to local conditions and will require the active cooperation of all concerned. The initiative for such a reorganization of casualty services cannot be supplied by the medical profession, especially surgeons and anesthetists, only. lt will have to emanate largely from Federal Authorities, from State (Land) Governments, from the communes, and, last but not least, from first-aid organizations. Instruction of laymen and doctors will have to be carried out with due regard being paid to the latest improvements of resuscitation technique, so that mistakes and dangers may be avoided.

Zusammenfassung

Die Chirurgie am Unfallort ist ein kleiner, aber wichtiger Teil einer modernen zeitgemäßen Unfallrettungsorganisation. Die Indikation zum Einsatz des Chirurgen sind lebensbedrohliche Zustände aller Art. Die Maßnahmen sind: 1. Respiratorische Reanimation, 2. kardiale Reanimation, 3. zirkulatorische Reanimation. Die Organisation ist den örtlichen Verhältnissen anzupassen und bedarf einer positiven Zusammenarbeit aller Beteiligten. Die Initiative zur Reorganisation des Unfallrettungsdienstes kann nicht nur von uns Ärzten, insbesondere Chirurgen und Anaesthesisten, ausgehen. Sie muß vorrangig ein Anliegen des Bundes, der Länder und Gemeinden, nicht zuletzt der Rettungsorganisationen sein. Die Ausbildung der Laien und Ärzte muß unter Berücksichtigung neuer Erkenntnisse in der Reanimation erfolgen, nur dann sind Fehler und Gefahren zu vermeiden.
