Summary
A systematic study on the behaviour of pulmonary tissue after temporary ischemia of various duration produced the following results:
With normothermia damage to the pulmonary tissue only occurs after ischemia for 1 hour. Initially this manifests itself as pulmonary emphysema which is the more pronounced the longer the ischemia persists. A reversible disorder of pulmonary gas exchange was observed after ischemia for 1–3 hours. If the ischemia persists for more than 3 1/2 hours either massive edema or a. hemorrhagic total necrosis develops. With ischemia of up to 6 hours the edema can be survived with difficult healing of pulmonary lesions. The reason for the relatively high ischemic tolerance is the comparatively low energy requirement of pulmonary tissue and the anerobic metabolism which persists up to 5 hours. lt was possible to confirm this by determining a few products of metabolism in the tissue homogenate.
Zusammenfassung
Systematische Untersuchungen über das Verhalten des Lungengewebes nach einer temporären Ischämie von verschieden langer Dauer führten zu folgenden Ergebnissen:
Erst ab einer Ischämie von 1 Std Dauer treten bei Normothermie Schäden am Lungengewebe auf. Diese äußern sich zunächst in einem Lungenemphysem, das urn so stärker ausgeprägt ist, je länger die Ischämie anhielt. Nach einer Ischämie von 1–3 Std wurde eine reversible Störung des Gasaustausches in der Lunge festgestellt. Bei Überschreiten einer Ischämie von 3 1/2 Std Dauer tritt entweder ein massives Ödem auf oder eine hämorrhagische Totalnekrose. Das Ödem kann bis zu einer Ischämie von 6 Std mit schwerer Defektheilung vom Lungengewebe überlebt werden. Ursache der relativ großen Ischämietoleranz ist der vergleichsweise geringe Energiebedarf des Lungengewebes und der bis zu 5 Std anhaltende anaerobe Stoffwechsel, was durch Bestimmung einiger Metabolite des Energiestoffwechsels im Gewebehomogenat nachgewiesen werden konnte.
