Summary

Owing to optimal frequency characteristics, electromanometry of the common bile duct permits the recording of rapid sphincter contractions. A precondition for the reproducibility of results is the methodical application of the rules of flow. During manometric or radiological examinations there occurs a stretching of the sphincter owing to the administration of fluid, and so a myogenic stimulation in the smooth muscle. A spontaneous activity, characteristic of many smooth muscles even in vitro, occurs which expresses itself mechanically as spasm, peristalsis or reduction of tonus. Normal evacuation of bile into the intestine occurs without measurable pressure changes. Peristalsis or papillar activity during examination are produced by exceeding the physiological amount of flow or pressure. if they occur with normal flow and normal pressure in the common bile duct they are a symptom of mechanical or inflammatory sphincter irritation.

Zusammenfassung

Vermöge optimaler Frequenzcharakteristik erlaubt die Elektromanometrie des Choledochus die Registrierung rascher Sphincterkontraktionen. Voraussetzung für Reproduzierbarkeit der Ergebnisse ist die methodische Anwendung der Strömungsgesetze. Bei manometrischen oder röntgenologischen Untersuchungen kommt es infolge Flüssigkeitszufuhr zur Sphincterdehnung und damit zur myogenen Erregungsbildung in der glatten Muskulatur. Es tritt eine für viele glatte Muskel auch in vitro charakteristische spontane Aktivität auf, die sich mechanisch als Spasmus, Peristaltik oder Tonusverminderung äußert. Die normale Galleentleerung in den Darm erfolgt ohne meßbare Druckschwankungen. Peristaltik bzw. Papillenspiel werden bei der Untersuchung durch Überschreiten der physiologischen Durchflußmenge oder des Druckes erzeugt. Wenn sie auch bei normaler Durchflußmenge und normalem Choledochusdruck gefunden werden, sind sie ein Symptom für mechanische oder entzündliche Sphincterirritation.
