Mechanische und biochemische Maßnahmen zur Thromboembolieprophylaxe nach Prostatektomien

Summary

Because of the increased liberation into the circulation of thromboplastic material and plasminogen-activating kinases in prostate operations a combination of anticoagulants and antifibrinolytics was given in 100 cases of prostatectomy as prophylaxis against thrombo-embolism. Heparin was used as anticoagulant and epsilon-aminocaprylic acid as antifibrinolytic. An increase of secondary haemorrhage to be anticipated after prostatectomy could be prevented by insertion of a double balloon catheter into the prostate bed, acting by mechanical compression of vessels and considerable protection of the wound against urine containing urokinase.

Zusammenfassung

Wegen des bei Eingriffen an der Prostata vermehrt freigesetzten und in die Blutbahn gelangenden thromboplastischen Materials und plasminogen aktivierender Kinasen wurde bei 100 Prostatektomien zur Thromboembolieprophylaxe eine kombinierte Anwendung von Antikoagulantien und Antifibrinolytica durchgeführt. Als Antikoagulans wurde Heparin benutzt und als Antifibrinolyticum die Epsilon-Aminocapronsäure. Mit Einlegen eines Doppel-Ballonkatheters in die Prostataloge konnte durch mechanische Kompression der Gefäße und weitgehende Abdichtung des Wundbettes gegenüber urokinasehaltigem Urin eine Verstärkung der nach Prostatektomien gefürchteten Nachblutungen vermieden werden.
