Nebenwirkungen der intraarteriellen cytostatischen Therapie bei fortgeschrittenen Tumoren im Kiefer- und Gesichtsbereich

Zusammenfassung

Die Verfasser berichten besonders unter Berücksichtigung der Komplikationen über die Möglichkeiten und Grenzen der intraarteriellen Chemotherapie von inoperablen malignen Tumoren im Kiefer- und Gesichtsbereich.

Versuche mit dem Cytostaticum Trenimon wurden aufgegeben, da nach intra-arterieller Applikation schwere lokale Gewebsschäden auftraten.

Beim Plattenepithelcarcinom der Mundhöhle hat sich die intraarterielle Methotrexattherapie am wirksamsten erwiesen.

Patienten mit Nieren- und Leberschäden und Patienten in reduziertem Allgemeinzustand sind für diese Therapie nicht geeignet, besonders wenn sie mit anderen Cystotatica vorbehandelt worden sind.

Abschließend wird diskutiert, ob die in 3 von 59 Fällen beobachteten tödlichen Komplikationen in Zukunft durch Knochenmarkstransplantation und durch die Verabfolgung von Thrombocytenkonzentraten vermeidbar sind.

Summary

The authors report on the possibilities and limitations of intraarterial chemotherapy of inoperable malignant tumors in the jaw and facial region, with special consideration of complications. Trials with the cytotoxic agent, Trenimony were abandoned because of the occurrence of severe local tissue damage after intraarterial administration. In squamous cell carcinoma of the oral cavity intraarterial Methotrexat therapy has proved the most effective. Patients with kidney and liver damage and those in poor general condition are not suitable for this treatment, especially if they have already been treated with other cytostatics. Finally it is discussed whether fatal complications, observed in 3 out of 59 cases, can be prevented in future by bone marrow transplantation and by administration of thrombocyte concentrates.
